Breast implant removal, or explant surgery, takes out breast implants and, when needed, the scar capsule around them. People choose it because of a hardened capsule, a rupture, pain, a wish to change size or simply no longer wanting implants, which are not lifetime devices. In Turkey, packages typically cost €2,500–€6,000, depending on the capsule work and any lift or new implants, with 5–7 days in the country.

What explant surgery is and how it works

Every implant sits in a capsule of scar tissue that is usually thin and soft but can thicken, harden or calcify. The surgeon removes the implant through an incision in the breast crease or around the areola and decides, ideally in advance, how much of the capsule to take. Leaving a healthy capsule means less dissection and a lower risk of bleeding; removing part or all of it (capsulectomy) is more extensive but sometimes necessary. The same operation can include a lift, fat transfer for volume, or new implants.

Capsular contracture, rupture and BIA-ALCL

Capsular contracture is graded I–IV on the Baker scale; grades III and IV (a firm, distorted or painful breast) are severe and may need surgery, and contracture can recur. Silicone ruptures are often silent, so MRI, or ultrasound as an alternative, is used to check. BIA-ALCL is a rare lymphoma of the immune system, not breast cancer, that develops in the fluid or capsule around an implant, more often with textured implants, usually years later as persistent swelling, a lump or pain. The FDA does not recommend removing implants in people without symptoms, but confirmed cases need removal of the implant and the capsule. Rare capsule cancers such as squamous cell carcinoma have also been reported.

Breast implant illness: what the evidence says

Some people link fatigue, joint pain, 'brain fog', anxiety and other symptoms to their implants, often called breast implant illness (BII). The FDA says these symptoms are poorly understood, and the UK regulator (MHRA) says it is not known whether implants cause them; there is no diagnostic test. In a prospective study funded by the Aesthetic Surgery Education and Research Foundation, 88% of women with such symptoms reported at least partial improvement a year after explant, whatever type of capsulectomy was done; the authors noted that lower expectations of harm may contribute. Removal can be reasonable, but no one can promise it will resolve symptoms.

Removing the whole capsule is a bigger operation, and a very thin capsule stuck to the chest wall can make en bloc removal difficult. The choice should follow the diagnosis, not marketing.

Who is a good candidate

  • Anyone with a confirmed or suspected rupture on MRI or ultrasound

Without symptoms, removal is rarely urgent: the FDA does not advise removing textured implants — including the Allergan Biocell devices recalled in 2019 — just because of the BIA-ALCL risk. If you notice new swelling or a lump years after surgery, have it investigated first, because the result can change which operation you need.

The procedure step by step

Records and imaging

Bring your implant card or operation notes (manufacturer, model, surface) and recent MRI or ultrasound reports; any late swelling should be tested before surgery.

Planning the capsule and shape

You agree in advance on removal only or a partial, total or en bloc capsulectomy, and whether a lift, fat transfer or new implants are part of the plan.

Anaesthesia

Usually general anaesthesia; a simple removal is sometimes done under intravenous sedation.

Removal

The implant and planned capsule are removed through a crease or areola incision, which may be longer for en bloc work; tissue can be sent for pathology.

Closure and drains

The pocket is closed, drains may be placed and you wake in a support bra, often going home the same day or the next.

Recovery timeline

    Tightness and soreness are expected. Drains, if placed, collect fluid from the old implant pocket.

    Drains out, first check

    Drains are usually removed within the first week, and the surgeon clears you to fly after a wound check.

    Most people return to office work within one to two weeks — longer after en bloc surgery or a lift.

    Heavy lifting and upper-body workouts restart gradually once your surgeon agrees.

    The breasts soften and reshape as the tissue adapts; the final result can take up to a year.

Results and how long they last

Without implants, breasts are usually smaller and flatter and may sag; long-standing implants can thin the overlying tissue, and dimpling or a hollow at the chest wall is possible. The result depends on how much natural tissue you have, the size of the old implants and the state of the capsule. Many people are happy with a natural look; others add a breast lift or fat transfer, sometimes a few months later once the skin has settled. Risks and how to reduce them

  • Bleeding (haematoma), fluid collection (seroma) and infection
  • Asymmetry, contour irregularities, dimpling and loose skin
  • Changes in nipple or breast skin sensation
  • Poor wound healing and unfavourable scarring, especially after a lift
  • Persistent pain, fat necrosis or, rarely, skin loss
  • Blood clots, anaesthetic risks and the possibility of further surgery

Late swelling? Get tested before you travel

A new, persistent swelling, lump or fluid collection a year or more after implant surgery should be assessed with imaging and fluid testing before any operation, because a confirmed BIA-ALCL diagnosis changes the surgery needed. Stop smoking, disclose all medication and do not fly home until your surgeon has examined you.
