Fat transfer — also called autologous fat grafting or lipofilling — moves your own fat from an area with some to spare, such as the abdomen or thighs, to one that lacks volume: most often the face, the breasts or the buttocks. It suits people who want a modest, natural-feeling change and accept that part of the fat will not survive. In Turkey a package typically costs €1,500–€6,000, depending on the area, and most patients plan 5–10 days in the country.

What fat transfer is and how it works

The operation has three stages. First, fat is removed with gentle liposuction, usually from the abdomen, flanks or thighs. Next, it is purified — by settling, filtering or centrifuging — to separate intact fat cells from fluid, oil and blood. Finally, the surgeon injects it through fine cannulas in many small droplets, so that each one lies close to a blood supply. Grafted fat has no blood vessels of its own at first: it must be nourished by the surrounding tissue until new vessels grow in, and cells that miss out die and are reabsorbed.

How much of the fat survives?

The American Society of Plastic Surgeons (ASPS) estimates that about 50% of fat injected into the face survives, and that the surviving fat is long-lasting. Studies vary widely: a systematic review found survival rates of 34% to 82% in the breast and 30% to 83% in the face, partly because volume was measured in different ways. Technique, the treated area, smoking and later weight changes all matter, so surgeons plan for some loss — and often for a second session.

Fat transfer to the buttocks is what most clinics market as a Brazilian butt lift (BBL): the same principle, but with specific safety rules covered below. Fat can also be added during a facelift or layered over implants in hybrid breast augmentation.

Who is a good candidate

  • Healthy adults with enough spare fat to harvest — very slim people may not have enough
  • People who want a modest, natural-looking change rather than a dramatic one
  • Non-smokers, or smokers willing to stop for several weeks before and after surgery — nicotine reduces blood flow to the grafts
  • People at a stable weight, because surviving fat cells grow or shrink as your weight changes
  • Anyone who accepts that more than one session may be needed

When fat transfer is not the right choice

  • You have little fat to spare, or plan to lose a lot of weight soon
  • You have an unexplained breast lump or are due for breast screening — have it checked first

  • You want a quick, reversible trial of a new look — for the face, dermal fillers may be a sensible first step

The procedure step by step

Consultation and planning

The surgeon checks your health and donor areas and discusses realistic volumes; before breast grafting, recent breast imaging may be requested.

Anaesthesia

Small facial grafts can be done under local anaesthesia with sedation; breast and buttock grafting usually need general anaesthesia.

Harvesting

Fat is removed by gentle liposuction through small incisions, using low suction to protect the cells.

Processing

The fat is purified and loaded into small syringes.

Injection

Small amounts are placed at several levels and angles. In the buttocks, fat must stay in the layer under the skin, above the muscle.

Dressings and discharge

The tiny incisions are closed or taped and a compression garment is fitted. Most patients go home the same day or after one night.

Recovery timeline

  1. Days 1–3

    Swelling and bruising

    Both donor and treated areas are swollen and bruised; the liposuction site is often the sorest. Walking from day one helps circulation.

  2. Days 5–10

    Check-up and flight home

    The NHS advises waiting 5–7 days after liposuction or breast surgery, and 7–10 days after facial procedures, before flying.

  3. Weeks 2–3

    Back to routine

    Most people return to desk work. After buttock grafting, surgeons usually restrict sitting directly on the buttocks for a while.

  4. Weeks 4–6

    Exercise

    A gradual return to sport once cleared; keep wearing compression garments as instructed.

  5. Months 3–6

    The result settles

    Fat that did not survive has been reabsorbed; what remains is broadly your long-term result and shows whether a touch-up is worthwhile.

Results and how long they last

Once grafted fat has a blood supply, it behaves like the fat around it, so the volume still present after about three to six months is generally long-lasting. It still ages with you and follows your weight: gaining weight can enlarge treated areas, and losing a lot can shrink them. Because survival is unpredictable, many patients need more than one session; the ASPS notes that multiple procedures are typically needed for the face. Results feel softer than implants, but the change is more modest and less precise.

Risks and how to reduce them

  • Partial fat loss — some grafted fat is always reabsorbed, which can leave the result smaller or uneven
  • Oil cysts and fat necrosis — dead fat can form firm lumps or oily cysts; most are harmless but may need a scan, drainage or removal
  • Calcifications in the breast that can show on mammograms — tell the radiologist you have had fat grafting
  • Asymmetry and contour irregularities in the treated or donor area
  • Infection, bleeding, prolonged swelling and numbness around the liposuction sites
  • Fat embolism — rare but potentially fatal if fat enters a blood vessel: in the buttocks when fat is injected into or under the muscle, and in the face when it enters an artery, where vision loss and stroke have been reported

BBL safety: fat only under the skin

Deaths after gluteal fat grafting have been linked to fat injected into or beneath the buttock muscle, where large veins run. Following a 2017 ASERF task force report, a multi-society task force of US plastic surgery bodies — including the ASPS and The Aesthetic Society — advises placing fat only in the layer under the skin, above the muscle, and supports ultrasound guidance to confirm that plane. Ask your surgeon exactly how they do this.

Judge photos by their timing

Swelling makes early results look fuller than they will stay. Ask for before-and-after photos taken at least three to six months after grafting, when the surviving volume is clear.

Fat transfer is less about how much is injected than about how much survives. A careful surgeon places small amounts in the right layer, plans honestly for a possible second session and, in the buttocks, never trades safety for size.
