A female hair transplant moves healthy follicles from a stable donor zone — usually the back and sides of the scalp — to areas that have thinned, such as the parting, the crown or a high hairline. It can work very well for the right woman, but fewer women are good candidates than men, so a proper diagnosis comes before any graft count. What a female hair transplant is and how it works

The surgeon harvests follicular units — natural groups of one to four hairs — from the donor area, where follicles are less affected by pattern hair loss, and implants them into tiny incisions in the thinning zone. Transplanted hair keeps the characteristics of its origin, so a stable donor gives lasting growth. The catch: hair loss in women is often diffuse, spreading across the top of the head and sometimes into the donor zone, which limits how much hair can safely be moved. Why diagnosis comes first

Female pattern hair loss typically shows as a widening parting and thinning on top, while the front hairline is usually preserved. But thyroid disease, low iron, hormonal conditions such as polycystic ovary syndrome (PCOS), childbirth, stress, some medicines, tight hairstyles and alopecia areata can cause similar hair loss — and a transplant fixes none of them. A dermatologist should first examine your scalp, often with trichoscopy (a magnified scalp exam), and order blood tests where needed, such as thyroid function, ferritin and hormone levels; a scalp biopsy is occasionally required.

Who is a good candidate

Usually not suitable: diffuse thinning that also affects the donor area, active alopecia areata or inflamed scarring alopecia, untreated thyroid or iron problems, and expectations of the density you had at twenty. Surgery is normally postponed during pregnancy and breastfeeding.

The procedure step by step

Diagnosis and remote planning

You send photos of your parting, hairline and donor area, plus recent blood results. A specialist checks the likely diagnosis and whether your donor area can support the plan.

The doctor examines your scalp, measures donor density and confirms the hairline or density map, technique and graft estimate in writing.

Harvesting under local anaesthetic

Grafts are extracted from the numbed donor area — between existing hairs in unshaven FUE — or a thin strip is removed for FUT. You stay awake throughout.

Implantation

Grafts are placed at natural angles, with single hairs at the front and along the parting. A session often takes 6–9 hours; large unshaven cases may be split over two days.

First wash and flight home

The clinic checks the grafts and shows you how to wash your scalp, usually the next day. Most women fly home 1–2 days later with written aftercare.

Recovery timeline

  1. Days 1–3

    Early healing

    Mild swelling, tenderness and small crusts around the grafts are normal. Sleep with your head raised and don't touch the implanted area; long hair covers the donor zone.

  2. Days 4–14

    Crusts clear

    Gentle daily washing clears the crusts by about day 10. Most women return to desk work within 3–7 days; strenuous exercise waits about two weeks, and FUT stitches come out at 10–14 days.

  3. Weeks 2–8

    Shedding phase

    Transplanted hairs usually shed — this is expected. Some women also notice temporary shedding of nearby native hair (shock loss), which normally grows back.

  4. Months 3–6

    New growth

    Fine new hairs appear, thin at first and then gradually thicker.

  5. Months 12–18

    Final result

    Density and texture mature; the final result is usually judged at 12–18 months.

Results and how long they last

With a stable donor area, transplanted hair is generally long-lasting. A good result means fuller coverage along the parting or a softer, lower hairline — not the density of your teenage years. Because female pattern hair loss tends to continue, the native hair around the grafts can keep thinning, so many specialists recommend medical treatment alongside surgery. Topical minoxidil (2% or 5%) is the most-recommended treatment for female pattern hair loss; it takes 6–12 months to work and only helps while you use it. Prescription tablets such as spironolactone help some women but can harm an unborn baby, so they need specialist supervision and reliable contraception. PRP hair treatment is widely offered, but its evidence is mixed.

Risks and how to reduce them

  • Shock loss — temporary shedding of existing hair near the grafts, more noticeable with diffuse thinning; it usually recovers within months
  • Poor growth if grafts go into skin with active, undiagnosed hair loss or are packed too tightly
  • Donor-area thinning from over-harvesting, a real risk when the donor zone is already fine
  • Forehead swelling, temporary numbness, itching, folliculitis (small spots) and, rarely, infection
  • Scarring: a fine line after FUT or small dots after FUE, usually hidden by long hair
  • An unnatural hairline or hair direction if the design ignores female hairline shape

A transplant does not treat the cause

If your hair loss comes from a thyroid problem, low iron, a hormonal condition or active alopecia areata, surgery will not stop it, and grafts placed into affected skin may fail. Be wary of clinics that book you in without examining your scalp or reviewing your health and blood results.
