SMILE is the newest of the mainstream laser vision procedures, and Turkey's high-volume eye clinics have made it far more affordable than in the UK. Its main draw is that it is flapless and minimally invasive — appealing to people put off by the idea of a corneal flap, and often gentler on eyes prone to dryness. As always, though, whether it is right for you depends on a proper eye examination, not on the marketing.
What is SMILE and how does it work?
SMILE (Small Incision Lenticule Extraction) uses a single femtosecond laser to shape a small disc of tissue — a lenticule — inside the intact cornea, then removes it through a tiny keyhole incision of just 2–4 mm. Because no hinged flap is created and the corneal surface stays largely intact, more of the eye's nerves and structural strength are preserved. It is used mainly to correct myopia and myopic astigmatism.

SMILE mainly treats short-sightedness; it is not the usual choice for significant long-sightedness.

- Adults over 18 with a stable prescription for at least 12 months
- Myopia (short-sightedness), with or without astigmatism, within the treatable range

Comprehensive eye assessment

Numbing drops
Anaesthetic drops are applied; the procedure is done awake with no injections and no general anaesthesia.
Shaping the lenticule
A femtosecond laser precisely shapes the lenticule inside the cornea in under 30 seconds per eye.
Keyhole removal
The surgeon removes the lenticule through the small incision, changing the cornea's focusing power.
Self-sealing incision
The tiny incision seals on its own; no flap and no stitches are involved.

Results and how long they last
For well-selected myopic patients, SMILE usually delivers a lasting reduction in dependence on glasses, and most patients achieve driving-standard vision or better. The tissue change is permanent. Two honest points remain: vision can take slightly longer to sharpen fully than with LASIK, and SMILE does not halt age-related presbyopia, so reading glasses are still commonly needed from the mid-forties. Enhancement is possible but handled differently than with flap-based surgery.
Cost of SMILE in Turkey and what's included
Headline prices in Turkey typically run from €1,500 to €3,000 for both eyes. SMILE tends to cost a little more than standard LASIK because of the specialised laser and single-platform technique. Turkey is cheaper mainly due to lower operating costs, exchange rates and high volumes — not lower-grade technology, since the laser systems are the same ones used in Europe.

Choosing a clinic safely
- Confirm the surgeon is an experienced ophthalmologist who performs SMILE regularly
- Ask how many SMILE cases they carry out — it is a more specialised technique than LASIK
- Insist on corneal topography and a full exam before any price is agreed
- Make sure the clinic will see you for the Day 1 follow-up in person before you fly
- Ask what happens if an enhancement is needed, given SMILE's flapless approach
- Avoid providers who quote a fixed price without examining your eyes
Red flag: SMILE 'sold' as suitable for everyone
SMILE is excellent for many short-sighted patients, but it is not the right answer for every eye — significant long-sightedness and certain corneal findings are better treated another way. Be cautious if a clinic pushes SMILE without a topography scan, or promises it regardless of your prescription. A trustworthy surgeon will sometimes tell you a different procedure, or no surgery, is the safer choice.
