Medical professionals conducting a hair transplant in a clinic setting

A hair transplant relocates hair follicles from the permanent zone at the back and sides of your scalp to areas that have thinned or gone bald. Because those donor follicles are genetically resistant to the hormone that causes pattern hair loss, they keep growing in their new position. It suits men and women with a stable, well-defined pattern of loss and a healthy donor area — it does not stop ongoing thinning elsewhere, and it cannot create new hair out of nothing.

How the procedure works

The whole session is done under local anaesthesia — you are awake, and after the initial injections settle you feel pressure and movement rather than pain. A typical case takes 6–8 hours, sometimes split across a day. The surgeon first cleans and numbs the scalp, then follicular units are extracted one by one from the donor area with a 0.7–1.0 mm micro-punch. Recipient sites are opened at a specific angle and direction, and the grafts are placed into them. The order of those last two steps is what separates the main techniques.

1

Planning and hairline design

The surgeon marks the new hairline and recipient zone with you, agrees a graft estimate, and photographs the donor and recipient areas.

2

Extraction (harvesting)

Individual follicular units are removed from the donor area with a micro-punch and kept in a chilled holding solution to protect them.

3

Channel opening

Tiny incisions are made in the thinning area at the correct angle, direction and density to mimic natural growth.

4

Implantation

Grafts are placed into the channels one by one. Single-hair units go at the hairline, multi-hair units behind them for density.

FUE, Sapphire FUE and DHI

Almost every modern transplant is a form of FUE (follicular unit extraction) — grafts are harvested individually rather than by cutting a strip of scalp. The variants differ mostly in the tools used for the recipient site. Sapphire FUE opens the channels with sapphire-tipped blades instead of steel, which allows finer, denser incisions. DHI (Direct Hair Implantation) uses a Choi implanter pen that opens the site and sets the graft in a single motion, so no separate channels are cut first. None is universally superior; a good surgeon recommends one based on your donor, the area treated and how dense the packing needs to be.

Sapphire refers to the blade used to open channels; DHI changes how grafts are implanted, not how they are extracted.

Grafts, the donor area and planning coverage

A graft is not a single hair: each follicular unit naturally carries 1 to 4 hairs, so a 3,000-graft session moves roughly 6,000–7,000 actual hairs. Your donor area is a fixed, lifetime resource — harvesting too aggressively thins the donor itself and cannot be undone. Planning balances three things: your degree of loss (measured on the Norwood scale), the density the surgeon can safely place (commonly 40–50 grafts per cm²), and preserving enough donor for any future work. Hairline design matters as much as graft count: a natural hairline is slightly irregular, age-appropriate and angled forward, not a straight low line.

Scars and how natural it looks

FUE leaves tiny dot scars across the donor area rather than the single long line of the older strip (FUT) method. Once the surrounding hair grows to a few millimetres these dots are usually invisible, though very short haircuts can reveal a slight stippling. Whether the final result looks natural depends less on the machine and more on angle, direction, density and hairline design — which is exactly where an experienced operator matters. Poorly angled, over-dense or too-straight work is the signature of a rushed, technician-only job.

Recovery: what the first year looks like

  1. Days 1–3

    Immediately after surgery

Small crusts form around each graft; the donor area is dressed. Forehead swelling and mild soreness are normal and settle within a few days.

  2. Weeks 1–2

    Scabbing and first wash

You follow a gentle washing routine. Scabs fall away and redness fades; grafts are secure by around day 10.

  3. Weeks 2–4

    Shock loss (shedding)

The transplanted hairs shed. This alarms most patients, but the follicle stays alive under the skin — only the visible shaft falls. Shedding is expected, not a failure.

  4. Months 3–4

    New growth begins

Fresh hairs start pushing through, thin and fine at first. Growth is uneven at this stage and improves month by month.

  5. Month 8

    Around 80% of the result

By roughly month eight most of the transplanted hair is through and thickening, and the shape of the result is clear.

  6. Months 12–18

    The final result

Density, texture and the hairline mature and settle. Crown work and DHI cases can take the full 18 months.

What a hair transplant costs in Turkey

Turkey is cheaper mainly because of lower clinic overheads, a favourable exchange rate and very high patient volume, not lower surgical standards — the leading clinics operate to international norms. Price is usually quoted per graft or as an all-in package. The figures below are approximate; your real quote is set only after a proper assessment of your donor and degree of loss.

FactorHow it affects the price
Graft countThe main driver. More grafts (larger area or advanced loss) cost more, whether priced per graft or by package.
TechniqueStandard FUE is usually the baseline; Sapphire FUE and DHI typically cost more for the finer tooling and time.
Surgeon involvementA surgeon-performed or surgeon-led procedure costs more than a technician-led one — and is worth verifying.
What's includedAll-in packages often bundle hotel, transfers, medication and aftercare; unbundled quotes can look cheaper but add up.

Prices are approximate and confirmed only after an individual assessment.

Choosing safely: surgeon vs technician

The single biggest quality question is who actually performs your surgery. Get the graft estimate, technique and named operator in writing, ask which steps the doctor personally does, and be wary of a firm graft number quoted before anyone has examined your scalp in detail. Treat '5,000 grafts in one day' at a rock-bottom price as a warning, not a bargain — mega-sessions strain the donor and counts are often padded. Compare at least three clinics on the same specifics, including the aftercare and revision policy if growth disappoints.

Who holds the punch?

In many high-volume clinics the surgeon marks the hairline and leaves, while unlicensed technicians perform the extraction and implantation for the rest of the day. Confirm in writing which steps a licensed physician performs personally. A named, verifiable surgeon who stays in the room is worth more than any package discount.

General information, not medical advice

This page is general information and not a substitute for a medical consultation. Whether a hair transplant is right for you, which technique fits, and how many grafts are realistic can only be decided after an individual assessment by a qualified doctor. Every WhichClinic listing is verified, but the clinical decision is always yours and your surgeon's.

A hair transplant is permanent — the good ones and the bad ones both last. The hour you spend verifying who will hold the punch matters more than the year you'll spend waiting for it to grow.
