The Brazilian Butt Lift (BBL) is one of the most requested body-contouring procedures for international patients in Turkey. It reshapes and lifts the buttocks using your own fat — liposuctioned from the waist and belly, then re-injected — so it slims and rounds in one operation. But the BBL also has one of surgery's most serious safety issues, and it hinges entirely on where the fat is placed. This guide is honest about both the appeal and the risk.
What a BBL is and how it works
A BBL is a fat transfer: a board-certified plastic surgeon first liposuctions fat from donor areas such as the abdomen, flanks and lower back, purifies it, and then carefully re-injects it into the buttocks to add volume and shape. Because it uses your own tissue, there is no implant. It is performed under general anaesthesia in an accredited hospital and typically takes 2 to 4 hours. The 'lift' effect comes from removing waist fat and adding projection — sculpting the whole lower silhouette, not just the buttocks.
BBL versus buttock implants
| Feature | BBL (fat transfer) | Buttock implants |
|---|---|---|
| Material | Your own fat | Silicone implant |
| Feel | Natural, soft | Firmer, can feel artificial |
| Also slims the waist | Yes, via liposuction | No |
| Key limitation | Some fat is reabsorbed | Higher long-term implant risks |
| Volume ceiling | Limited by available donor fat | Larger volumes possible |
Most surgeons prefer fat transfer where enough donor fat exists, for a more natural result.
Who a BBL is suitable for
- People with enough donor fat on the waist, belly or back to harvest and transfer
- Those wanting a rounder, lifted shape with a natural feel rather than an implant
- Non-smokers in good health, cleared for general anaesthesia
- Patients with realistic expectations — final size depends on how much fat survives
- Those able to avoid sitting directly on the buttocks for several weeks afterwards
The procedure step by step

Consultation & planning
The surgeon assesses donor fat, buttock shape and skin, and agrees a realistic volume and silhouette.
Anaesthesia
General anaesthesia given by an anaesthetist for a procedure lasting a few hours.
Liposuction harvest
Fat is removed from the waist, abdomen and back — this also sculpts your curves.
Purify & graft
The fat is processed, then injected in small amounts into the subcutaneous (above-muscle) layer only for safety.
Recovery & discharge
A garment is fitted; most stay one to two nights, then recover locally, avoiding direct pressure on the buttocks.
Recovery timeline
Days 1–3
No sitting on the buttocks
You lie on your front or side. Bruising and swelling in both donor and buttock areas are normal.
Days 5–7
Special cushion
Short periods sitting on a BBL pillow (weight on the thighs) may begin, per your surgeon's guidance.
Days 7–10
Fit to fly
Most patients travel home in this window once cleared, ideally sitting on a cushion — early flights raise clot risk.
Weeks 2–4
Fat 'takes'
Some transferred fat is reabsorbed; the surviving fat becomes your permanent result. Keep wearing the garment.
3–6 months
Final shape
Swelling resolves and the final volume and contour settle over three to six months.
Results and how long they last
A BBL gives long-lasting results because the surviving fat is living tissue that stays. Honestly, though, not all the transferred fat survives — typically a meaningful proportion is reabsorbed in the first months, which is why surgeons slightly over-fill and why final size is never exactly predictable. Big weight changes afterwards will alter the result, since the grafted fat behaves like fat anywhere. Some patients need a top-up session later. Realistic expectations and a stable weight are the keys to a lasting outcome.

Choosing your clinic safely
- Confirm a board-certified plastic surgeon performs the BBL — this procedure demands proven expertise
- Ask directly whether fat is injected above the muscle only and whether ultrasound guidance is used
- Insist on an accredited hospital with an anaesthetist, not an office setting
- Check the garment, BBL cushion, medication and follow-up are included in writing
- Walk away from clinics promising extreme size or refusing to discuss BBL safety technique
Safe grafting depth is a life-or-death detail
The BBL's serious risk is fat embolism — fat entering a large vein and travelling to the lungs, which can be fatal. This risk is tied almost entirely to depth: fat must be injected into the subcutaneous layer above the muscle, never deep into the gluteal muscle. Modern safe practice injects above the muscle, often with ultrasound guidance and a blunt cannula. Ask your surgeon exactly how they inject; if they can't answer clearly or dismiss the question, do not proceed.
Medical disclaimer
This guide is general information, not medical advice. Whether a BBL suits you, your donor fat, and your individual risks can only be judged by a qualified surgeon after an in-person assessment. Always seek a personal consultation before deciding.
With a BBL, technique is everything — a natural, lasting result comes from a surgeon who injects safely above the muscle, not from chasing the biggest possible size.
